Student Health Services Policy on Medical Information Release

If a student wishes to grant a parent access to their medical records, they must sign a Release of Medical Information form in person during their appointment. This release will be reviewed at each subsequent visit. Students retain the right to withhold private medical information even with a signed release. In the event a student becomes incapacitated, only then will a designated medical power of attorney be contacted.
